Indiana is a Direct Access state for Physical Therapy. You can see a Physical Therapist for 6 weeks without a referral. If your treatment extends beyond 6 weeks, then we will work to get a referral for Physical Therapy Services from an appropriate Healthcare Provider.
Referrals can come from your physician, podiatrist, chiropractor, dentist, nurse practitioner, or physician assistant.
If you have Medicare, you will need a referral from your doctor for us to get started.

I’m an out-of-network, cash-based physical therapist with clear prices for specialized care not restricted by insurance. I can provide a superbill for you to submit to your insurer, but reimbursement isn’t guaranteed. If you want help, use the insurance worksheet.
I am a provider for Medicare Part B and am able to bill Traditional and Advantage plans.
I am not credentialed with Indiana Medicaid and cannot provide or bill for skilled physical therapy services to Medicaid members at this time.
Wellness services are not considered “skilled” in the rules of insurance and are cash-based.
