Indiana is a Direct Access state for Physical Therapy. You can see a Physical Therapist for 6 weeks without a referral. If your treatment extends beyond 6 weeks, then we will work to get a referral for Physical Therapy Services from an appropriate Healthcare Provider.
Referrals can come from your physician, podiatrist, chiropractor, dentist, nurse practitioner, or physician assistant.
If you have Medicare, you will need a referral from your doctor for us to get started.

Insurance & Payment Information
For skilled Physical Therapy services, I operate as an out-of-network provider. This allows us to focus entirely on your treatment without the restrictions often imposed by private insurance. While I do not bill insurance directly, I can provide a "superbill" for you to submit for potential reimbursement; please note that reimbursement depends on your specific plan. To help you navigate this, I recommend using our provided Insurance Worksheet.
Medicare & Medicaid
Medicare: I am a Medicare Part B provider and bill both Traditional and Advantage plans. As I am out-of-network for Advantage plans, I encourage you to verify your specific physical therapy benefits.
Medicaid: I am not currently credentialed with Indiana Medicaid for skilled physical therapy. However, Medicaid members are welcome to join my cash-based Wellness Services.
Wellness Services
Wellness services are designed for proactive health and coaching. These are not billed as "skilled" therapy under insurance rules and are provided on a flat-rate, cash-pay basis. In some instances, my group classes are provided free of charge by the organization hosting the class.
